Duties

Provide education to patients and their families regarding treatment plans and home exercises.

Participate in discharge planning to ensure a smooth transition from therapy to home or other care settings.
Maintain accurate documentation of patient care and progress using appropriate medical terminology.
Qualifications
Degree in Physical Therapy from an accredited program.
Valid state license to practice as a Physical Therapist.
Experience in acute care settings, outpatient clinics, or pediatrics is preferred but not required.
Strong understanding of patient care systems and discharge planning processes.
Proficient in medical terminology relevant to physical therapy practices.
Excellent communication skills with the ability to build rapport with patients.
Ability to work collaboratively within a multidisciplinary team.
