Job Summary and Responsibilities

The Enhanced Referral /Family Recruitment Specialist is part of a team that is responsible for ensuring that families and children are provided with the resources they need to insure the healthy development of the child. The Specialist is responsible for connecting organizations for intake referral and participants in the CommonSpirit St. Joseph's Children (SJC) home visiting program with needed services.
  • Enhanced Referral: Works in partnership with home visiting staff to ensure that needs of family and children participating in the home visiting program are appropriately met.
  • Communicate effectively and regularly with home visitors. Provides regular shadowing / training opportunities for newly hired staff in the program.
  • Make referrals to programs and services, providing appropriate levels of support to insure successful referral.
  • Provide follow-up as needed to ensure referral was successful.
  • Effectively meet needs of parent/family and child respecting strengths and preferences of parent/family.
  • Document all enhanced referral tasks based on protocols.

Job Requirements

Required
  • Bachelors Other Bachelor’s Degree in Early Childhood Education, Social Work, Public Health or a similar discipline. or 8 years of direct relevant experience, upon hire
  • Driver's License: NM, upon hire and
  • Basic Life Support - CPR, within 60 Days
