About Us

Pemi-Baker Hospice & Home Health is a nonprofit agency that has been caring for patients and families in central New Hampshire since 1967. We provide home health and hospice care to patients in their homes throughout Plymouth and surrounding areas.

About the Role

We’re looking for a full-time Community Outreach Liaison who’s passionate about engaging with people and building relationships! In this role, you’ll be interacting with current referral sources, as well as potential new referral sources. This is a field-based role, so you’ll spend most of your time traveling throughout beautiful New Hampshire visiting hospitals, physician offices, and facilities.

What You’ll Do

• Build and maintain relationships with hospitals, physician practices, skilled nursing and assisted living facilities, discharge planners, and care managers
• Create and manage a territory plan with priority accounts and regular visit schedules
• Educate referral partners on our services, eligibility guidelines, and referral process
• Follow up on referral opportunities and hand them off smoothly to our intake team
• Track activity and results and review them regularly with the Executive Director

What You Bring

• 2+ years of successful experience in healthcare sales, referral development, account management, or a similar role
• Hospice, home health, or other post-acute experience preferred
• Professional connections in central New Hampshire a plus
• Organized, self-motivated, and good at follow-up
• Valid NH driver’s license, reliable transportation, and current auto insurance
• No clinical license required
