The Care Navigator is responsible for coordinating care for patients and caregivers, including scheduling appointments and explaining insurance benefits. They also perform clinical screenings, document patient information, and provide emotional support under clinical supervision.

About the Role
We’re looking for a Care Navigator to join our existing care team who has prior experience coordinating/navigating care for patients, caregivers, and their loved ones - bonus points if you’ve worked with a patient population who experience cognitive challenges. This role requires genuine empathy, patience, exceptional time management and communication skills.
You will spend >70% of your day on the phone connecting with and coordinating care for your patients and their caregivers. Additionally, you must be able to provide your own personal computer/laptop that meets security and telehealth standards (SOC2).
