What We’re Looking For

Trillium Health Resources has a career opening for a Care Manager – IW to join our Care Management Team! The Care Manager provides comprehensive, whole-person care management to members within a managed care environment. The role serves as the primary point of contact for assigned members, independently conducting assessments, developing care plans by following established guidelines, and coordinating services across providers and community resources to ensure continuity and quality of care, while ensuring alignment with regulatory and organizational requirements.
