Tandem Care is hiring a Care Coordinator to support our Continuous Care (CC) division—our highest-touch, most operationally complex service line.
This is not a traditional care management role focused primarily on assessments and check-ins. This is a fast-paced operational position that requires exceptional organization, strong judgment, and the ability to make decisions quickly while balancing the needs of clients, families, and caregivers.
Our Continuous Care clients receive dedicated, ongoing care ranging from 10 hours per week to 24/7 coverage. Because of that, this role is heavily focused on staffing execution, schedule management, service recovery, caregiver accountability, and keeping care running smoothly despite the inevitable curveballs that come with home care.

Serve as the primary point of contact for an assigned caseload of Continuous Care clients and families
Build and maintain caregiver schedules for ongoing cases, ensuring consistency and continuity whenever possible
Respond to caregiver call-outs, schedule disruptions, and urgent coverage needs in real time
Coordinate replacement coverage quickly while maintaining a positive client experience
Match caregivers to clients based on personality, skillset, availability, and family preferences
Conduct client onboarding calls and prepare caregivers thoroughly for new assignments
Coach caregivers on client-specific expectations, care plans, and performance concerns
Manage client concerns, complaints, and service recovery situations with professionalism and urgency
Proactively check in with families to identify changes in needs before they become larger problems
Collaborate closely with Recruiting, HR, Sales, and Leadership to ensure staffing pipelines support client demand
Review caregiver visit notes and identify trends, gaps, or opportunities for improvement
Document all communication, schedule changes, client updates, and action items accurately and consistently
Think critically and make judgment calls independently while escalating concerns appropriately
Some days will go exactly as planned.
Most won't.
A caregiver calls out thirty minutes before a shift. A family requests schedule changes. A client is discharged unexpectedly from rehab. Someone needs to be staffed immediately. A caregiver isn't the right fit. A family needs reassurance. Another client needs an additional overnight shift.
You'll pivot constantly.
You'll prioritize dozens of moving pieces.
You'll make decisions quickly.
And you'll have the satisfaction of knowing that because of your work, vulnerable seniors remain safe at home and families can breathe a little easier.
Have worked in home care operations and understand the realities of staffing hourly care
Can remain calm and solutions-focused during high-pressure situations
Actually enjoy solving scheduling puzzles
Can deliver difficult feedback to caregivers professionally and directly
Are highly structured and organized
Can juggle multiple priorities without dropping details
Have excellent follow-through and don't need reminders to close loops
Know how to balance empathy with accountability
Understand that responsiveness matters and that families remember how you show up during difficult moments
Take ownership of outcomes rather than waiting for someone else to solve problems
Prefer predictable, repetitive work
Become overwhelmed when priorities shift throughout the day
Dislike staffing and schedule management
Avoid difficult conversations with caregivers or clients
Need extensive direction before taking action
Are looking for a primarily administrative role with minimal phone interaction
2+ years of experience in care coordination, staffing, scheduling, client services, or operations within a licensed home care agency
Direct experience staffing ongoing caregiving cases and managing caregiver schedules
Experience handling caregiver call-outs and urgent service recovery situations
Strong working knowledge of caregiver-client matching principles
Exceptional communication skills across phone, email, and internal systems
Ability to work independently in a fully remote environment
Bachelor's degree preferred but not required
Competitive pay: $28–$30/hour, based on experience
