The Triage RN performs comprehensive initial and ongoing assessments of patients and families via phone to determine hospice needs. They develop care plans, coordinate patient care as a Case Manager, and provide professional nursing instructions and treatments.

The role involves prospectively reviewing OASIS assessments and ICD-10 coding to ensure accuracy and compliance with federal and state regulations. The nurse will collaborate with clinical teams to identify documentation gaps and participate in quality improvement activities.
