The role involves evaluating and managing members with urgent symptoms or treatment complications via telemedicine, developing comprehensive care plans in collaboration with the team, and providing clinical oversight for interdisciplinary case conferences. Responsibilities also include completing annual clinical intake visits for select members and participating in emergency coverage as needed.

The Nurse Practitioner will evaluate and manage members with urgent symptoms or treatment complications via telemedicine, developing comprehensive care plans and triaging care as needed. Additionally, they will develop care plans addressing clinical and non-clinical needs in collaboration with RNs and provide clinical oversight for interdisciplinary case conferences.

The Product Manager will lead Data Scientists and Analysts to generate insights for improving the care model, build data products to assess Care Team performance, and support partner outcome reporting. Responsibilities include embedding into Care Team workflows to solve high-impact problems and developing/maintaining a clear product roadmap.
