The Licensed Vocational Nurse will provide personalized nursing care, including medication administration and wound care, to patients in their homes. They are responsible for conducting health assessments, educating families, and documenting patient activities in electronic health records.

Assess patients to develop and implement personalized treatment plans with measurable goals. Prepare and submit clinical progress summaries while coordinating care with other medical professionals.

The Registered Nurse will conduct health assessments, develop individualized care plans, and provide direct patient care including medication administration and wound care. They will also educate patients and families while coordinating with an interdisciplinary team to ensure high-quality, holistic home health services.
