Social Services Representative, Advanced
Conduct telephonic assessments of members’ social service needs, provide care coordination and referrals to health plan and community resources, and support members after hospital or facility discharge. Document screenings and case notes, coordinate with internal teams and public agencies, make and follow up on mandated reports, and provide administrative support to the Social Services Management Team.
