86Borders is seeking an experience Nurse Practitioner who will lead and oversee the clinical outreach program to manage complex patients. This includes leading and supporting a team of nurses that are coordinating care for the patients, creating and reviewing patient care plans, and handling complex patient scenarios. This position reports to the Director of Care Coordination, with a dotted line to Chief Medical Officer.  

 

Required Experience and Qualifications:

  • Active, unrestricted Nurse Practitioner (NP) license in Texas; ability to obtain additional state licenses if needed.
  • Must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared.
  • Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
    • The Accreditation Commission for Education in Nursing (ACEN) or
    • The Commission on Collegiate Nursing Education (CCNE).
  • 5+ years of progressive leadership experience managing a team
  • 2+ years of remote work experience
  • Experience in Chronic Care Management
  • Dedicated home office for remote work
  • Experience working with Veterans and/or Medicaid and/or Dual Eligible patients
  • Strong interpersonal, communication (both verbal and written), and problem-solving skills
  • Experience working with customers over the phone and by text message
  • Experience documenting case notes in a care management or electronic health record platform
  • Experience with motivational interviewing

 

Preferred Experience and Qualifications:

  • Additional licensure in one or more of the following states is preferred: Louisiana, Mississippi, Arkansas, Oklahoma, Missouri, Kansas, Nebraska, Utah, Wyoming, Colorado
  • Preferable provider types include Adult Nurse Practitioners, Family Nurse Practitioners, Psychiatric Mental Health Nurse Practitioners (PMHNP), and Clinical Nurse Specialists (CNS)
  • Experience with Apple computers (i.e., MacBook) preferred
  • Bilingual fluency in Spanish preferred

 

Key Responsibilities:

  • Build care plans for complex patients
  • Oversee nurse outreach to complex patients
  • Handle clinical escalations
  • Collaborate with nurses, care coordinators, specialists, pharmacists, and other healthcare professionals to optimize patient outcomes
  • Partner with the Chief Medical Officer and other clinical staff to develop and enhance clinical operations
  • Partner with the Director of Care Coordination to ensure effective onboarding of team members and ongoing development of existing team members

Schedule:

  • Full-time, Monday - Friday, 8:30am - 4:30pm CST


Work Environment and Physical Requirements

  • This position is entirely remote, working from a dedicated home office.
  • This position requires sitting for long periods of time.
  • Travel is not required.
