About Us

Tend Health is a New Zealand-owned and independent primary healthcare provider with a vision to transform health outcomes for current and future generations. Tend combines the best of clinical care, technology, and people to deliver a patient and whānau experience that's convenient, accessible, and focused on the full spectrum of preventative, proactive, and acute care needs.

Why This Role Matters

As Senior Coordinator, you'll be at the heart of our mental health and addiction service, helping people achieve their goals, plan for a better future, and reduce the impact mental illness has on their lives and the lives of their whānau.

You'll lead the day-to-day rhythm of a multi-disciplinary care team, guide people smoothly through transitions across the service system, and provide a calm, confident presence for your colleagues.

What You'll Be Doing

Care Coordination & Clinical Practice

You'll facilitate patient transitions across the service system, keeping care continuous and high quality. You'll triage referrals and match them with the right team members for discipline-specific assessment. You'll identify care needs, coordinate interventions, and evaluate outcomes — working autonomously with patients presenting complex needs across mental health and addiction services, and coordinating holistic support plans from assessment through to resourcing.

Leadership & Service Development

You'll provide operational and people leadership for the mental health team, and play an active role in shaping and refining the role itself as the service grows. You'll support and mentor your colleagues, bring analytical thinking to service development and evaluation, and contribute to wider initiatives, working groups, and research. Championing equitable, culturally safe care will be second nature to you.

Quality & Information

You'll keep documentation accurate, timely, and to a high standard, meet deadlines with consistency, and ensure practice aligns with all relevant regulatory and professional expectations.

What You'll Bring

Essential

  • Current registration and an Annual Practising Certificate as a Registered Nurse, Occupational Therapist, Social Worker, or Psychologist

  • Experience in a mental health, community health, or primary care setting

  • Experience working alongside Māori and Pasifika communities

  • A track record coordinating care for people with complex, severe, and enduring mental health and addiction needs

Desirable

  • Experience across both statutory and NGO mental health and addiction services

  • Experience within a multi-disciplinary and/or integrated care model

  • Experience delivering or coordinating care via telehealth or online platforms

The Person We're Looking For

You bring a whānau-centred, collaborative, partnership-based approach to everything you do. You communicate with warmth and clarity, think analytically, and solve problems with confidence. You're skilled at building the kind of working relationships that get things done, and you can manage competing priorities without losing sight of quality or accuracy. You're comfortable with technology and digital tools, and you're driven by a genuine, proactive commitment to better outcomes for tangata whaiora.
